Could be the wrong shape on the part of the seating die that pushes the bullet into the case. Some of them have sharp corners. You need one which matches the bullet It won't matter much at very short ranges but could make a big difference at longer ones.

Back when I had one, I had nothing but trouble with my lee die's,
Changed to RCBS & that was that.
now even more baffled, just bought a different die and still the same?
